Job Description
The RN Navigator's primary responsibility is to examine patient care procedures with the aim of fostering effective partnerships with Radiologists, Physicians, and Medical Consultants. In this capacity, they function independently to facilitate communication between various stakeholders, drawing on their knowledge to establish, assess, and compile essential operational and clinical guidelines. Seeking input from senior management for complex cases is a standard practice in this role.
Essential Job Function:
- Evaluates patient instances, health records, and present healthcare approaches to adequately lead patients along their healthcare path.
- Works alongside diverse healthcare teams to devise comprehensive care plans tailored to meet the specific needs and preferences of each patient.
- Maintains accurate documentation of patient engagements, care plan progress, and navigational strategies for ensuring ongoing and coordinated healthcare services.
- Uses knowledge of healthcare systems, available resources, and patient needs to assist patients in maneuvering through their medical process, securing access to vital services and aid.
- Investigates patient-centered navigational methods and interventions.
- Establishes and upholds effective communication with both healthcare teams and patients to ensure a seamless flow of information and continuity in healthcare provision. Acts as a valuable asset in improving patient navigation services within the organization, occasionally reaching out to senior management for advice on challenging navigation issues.
- Undertakes supplementary responsibilities as delegated.
Education:
- Required: Associate's Degree in Nursing.
- Preferred: Bachelor's Degree in Nursing.
Experience:
- Required: 5 years as a Registered Nurse with 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification/Listing:
- Required: Registered Nurse license in North Carolina or a Compact state, BLS (CPR)-American Red Cross or AHA Healthcare Provider.
