The Clinical Social Worker II's primary responsibility is to deliver social work support to patients and their families to address both psychological and physical needs effectively. - In delivering social work services, the Clinical Social Worker II engages with patients and their families to fulfill psychosocial and clinical needs, while excluding PRN/Relief components. By leveraging their knowledge and practical know-how, this role effectively identifies and responds to the diverse needs of patients, both within and outside the clinical realm, using therapeutic methods and other clinical strategies.
- Selecting the patients to prioritize for clinical social work attention is essential in creating a suitable support system for both staff members and patients.
- Uses information gathered from both the patient and healthcare providers to gain insight into the patient's circumstances and facilitates communication among all parties to prevent issues caused by incomplete information.
- Uses clinical expertise and experience to guide patients towards community resources and opportunities for enhanced health care outcomes.
- Creating a care plan for patients with minimal involvement from clinical staff to assist them in receiving the necessary treatment. Providing guidance to staff, patients, and families on important care-related information to foster improved comprehension and acting as a bridge between all stakeholders. Designing a treatment strategy for patients with limited support from medical professionals to guarantee the delivery of suitable care. Teaching healthcare providers, patients, and families about key care details to enhance overall awareness and knowledge.
- Responds to critical situations by providing assistance and taking charge in high-pressure environments for staff, families, and patients, utilizing clinical experience to alleviate tensions and offer explanations that aid in defusing the anxieties of patients and their families.
- Provides clinical care services to patients and their families to address immediate and future psychosocial and medical needs.
- Undertakes a thorough assessment of patients' psychosocial backgrounds to effectively match them with the most suitable care options considering their medical and social needs.
- Employs diverse counseling and therapy techniques to alleviate any concerns or anxieties the patient and their family may have regarding medical treatments.
- Executes any other responsibilities as designated.

Required: Masters of Social Work

Required:
Minimum of two (2) years of recent experience in a health-related environmentÂ
