Job Description
The Case Manager Registered Nurse (RN) is responsible for offering thorough and harmonized patient care. Working under either moderate or general supervision, this position involves analyzing patient needs, engaging in care coordination, conducting research, reconciling information, and reviewing care plans.
Essential Job Function
- Assesses patient situations to understand their needs and establish tailored care schemes.
- Manages documentation tasks concerning patient care to ensure compliance with regulatory standards.
- Keeps abreast of the latest evidence-based practices and healthcare directives pertinent to the specified patient demographic to infuse recent insights into care strategies and interventions.
- Assesses the efficacy of care programs, monitors patient responses, and adjusts interventions when necessary to enhance patient health with the aim of reaching desired outcomes.
- Executes tasks that are assigned beyond regular duties.
Education
- Required: Associate's DegreePreferred: Bachelor's Degree
Experience
- Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
