Job Description
The Case Manager Registered Nurse (RN) delivers comprehensive and coordinated care to patients, with oversight ranging from moderate to general. Duties in this position encompass analyzing patient needs, engaging in care coordination, researching, reconciling information, and reviewing care plans.
Essential Job Function:
- Examines patient instances to evaluate their needs and devise personalized care schemes.
- Compiles and manages documentation linked to patient care to uphold accuracy and adherence to regulations.
- Engaging in ongoing research and maintaining awareness of evidence-based practices and healthcare standards applicable to the assigned patient population aids in integrating the latest insights into treatment approaches and interventions.
- Reviews and assesses the efficiency of care schedules, monitoring patient results and implementing changes as necessary to enhance patient wellness for the achievement of intended aims.
- Undertakes supplementary duties as instructed.
Education
- Required: Associate's Degree Preferred: Bachelor's Degree
Experience
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
