Responsibilities:

- Examines details provided by both the patient and clinical team to grasp the patient's status and conveys this information clearly to all involved parties to enhance coordination and address any issues stemming from inadequate information sharing.
- Through a combination of clinical insights and firsthand experience, the assistance facilitates connections between patients and community resources to ensure their healthcare needs are met with excellence.
- Create a structured care plan for patients with little assistance from healthcare providers to ensure they receive optimal treatment.
- Serves as a bridge by educating staff, patients, and families on important care-related information, contributing to improved awareness and comprehension of the situation within the healthcare setting.
- Provides support and takes a leadership role during times of high stress for employees, families, and patients by utilizing clinical expertise to alleviate tensions in difficult situations, offering explanations that help ease the worries of patients and their loved ones.
- Provides clinical care services for patients and their families with a focus on meeting both current and future psychosocial and medical needs.
- Undertakes a thorough psychosocial assessment of individuals to better comprehend their circumstances and match them with suitable care options based on their medical and social outlook.
- Employs a variety of counseling and therapy strategies to help the patient and their family feel at ease in relation to clinical and medical treatments.
- Undertakes supplementary duties as instructed.
Qualifications:
EDUCATION:
Required: Masters of Social Work
EXPERIENCE:
Required: Minimum of two (2) years of recent experience in a health-related environment
