Job Description
The Case Manager Registered Nurse (RN) offers thorough and synchronized care to patients, working under moderate or general supervision. In this role, responsibilities include evaluating patient needs, participating in care coordination, conducting research, aligning information, and scrutinizing care plans.
Essential Job Function:
- Assesses patient scenarios to understand their needs and formulate tailored care blueprints.
- Organizes and manages patient care forms to guarantee they meet regulatory standards.
- Keeping abreast of research findings and contemporary healthcare protocols associated with the designated patient group allows for the utilization of up-to-date knowledge in developing care plans and interventions.
- Examines and judges the success of treatment plans, overseeing patient advancements and adapting care procedures as required to maximize patient health outcomes in order to reach set targets.
- Carries out any other tasks as delegated.
Education:
- Required: Associate's degree.
- Preferred: Bachelor's degree.
Experience:
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification/Listing:
- Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider.
