Job Description
The RN Navigator's duties involve evaluating patient care to ensure efficient coordination among Radiologists, Physicians, and Medical Experts. Working with limited oversight, they function as a communication and information hub for clinical and administrative purposes, utilizing their expertise to create, evaluate, and organize necessary operational and clinical protocols. Guidance from senior management is sought for intricate cases by individuals in this role.
Essential Job Function
- Reviews patient instances, medical backgrounds, and current care protocols to adeptly navigate patients throughout their healthcare expedition.
- Collaborates with diverse medical professionals and departments to establish customized care plans that offer patients a well-coordinated and empathetic healthcare journey.
- Documents patient engagements, oversees the progression of care plans, and records navigational actions to guarantee steady and cohesive healthcare services.
- Employs familiarity with healthcare systems, available resources, and patient needs to lead patients along their medical journeys, ensuring they have access to essential services and assistance.
- Assesses interventions aimed at enhancing patient-centric navigation initiatives.
- Maintains open and transparent communication with both healthcare teams and patients, emphasizing the importance of clear information sharing and consistent care provision. Moreover, plays a pivotal role in advancing patient navigation services for the company and may approach senior management for support with complex navigation issues.
- Completes miscellaneous tasks as assigned.
Education:
- Required: Associate's Degree in Nursing.
- Preferred: Bachelor's Degree in Nursing.
Experience:
- Required: 5 years as a Registered Nurse with 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification/Listing:
- Required: Registered Nurse license in North Carolina or a Compact state, BLS (CPR)-American Red Cross or AHA Healthcare Provider.
