Job Description
Overview:
In this role, the Case Manager Registered Nurse (RN) administers comprehensive and harmonized healthcare to patients. Under moderate or general guidance, this position evaluates patient necessities, engages in care coordination, conducts inquiries, aligns data, and appraises care schemes.
Responsibilities:
Assesses patient instances to identify their needs and develop customized care plans.
Compiles and handles paperwork connected to patient healthcare to ensure precision and conformity with regulatory standards.
Stays current with evidence-based practices and healthcare recommendations applicable to the assigned patient population to enhance the effectiveness of care plans and interventions.
Reviews and judges the efficiency of healthcare plans, supervising patient developments, and adjusting interventions as required to enhance patient welfare and achieve desired outcomes.
Undertakes supplementary duties as instructed.

EDUCATION:
Required:
- Associate's Degree
Preferred:
- Bachelor's Degree
EXPERIENCE:
Required:
- 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIRED
Required:
Registered Nurse license in North Carolina or a Compact state
BLS (CPR)-American Red Cross or AHA Healthcare Provider
