Job Description
As a support staff for Case Managers and Social Workers, the Care Management Assistant II plays a crucial role in maintaining the precision of patient and insurance data, expediting reimbursement procedures, and overseeing the flow of patient admissions. Moreover, this position focuses on cultivating and upholding post-acute network affiliations throughout the Cone Health organization.
Essential Job Function
- Assists social workers and Case Managers with administrative tasks by handling paperwork and reaching out to patients, aiming to reduce the workload on clinical staff and improve overall efficiency within the office.
- Producing written materials, like referral paperwork, with accurate grammar and spelling is essential for effective communication with other professionals, whether they are within the organization or external.
- Communicates with patients, caregivers, insurance firms, post-acute suppliers, and community entities, under the supervision of a Case Manager or Clinical Social Worker (LSW).
- Maintaining the accuracy of information is essential.
- Providing department leaders with insights into potential coverage gaps for social workers facilitates the identification of resource needs and empowers leadership to devise strategic solutions to address the challenge at hand.
- Cultivates robust partnerships with the post-acute network associates of Cone Health in diverse care facilities such as SNFs, LTACs, IRFs, and long-term care settings.
- Building networks and supporting business expansion by liaising with post-acute stakeholders, VBCI leadership, and Cone Acute institutions to facilitate a reciprocal flow of feedback concerning referrals and educational prospects.
- Engages in a collaborative approach with our Cone Health acute care facility, Cone's post-acute resources, and community partners to execute and assess a high-quality patient admission journey.
- Carries out supplemental responsibilities as instructed.
Education
- Required: LPN training program or Bachelor of Social Work
Experience
- Preferred: 1-2 years in Care Management or Post-acute support
Licensure/Certification/Listing
- Required: Current LPN licensure as required by state or BSW degreeBLS
