Overview:
The Case Manager collaborates with specific, vulnerable groups to ensure they receive optimal care tailored to their requirements. This role involves identifying, evaluating, and managing cases, as well as predicting requirements associated with sickness, hospital admittance, financial matters, schooling, and services within the home or community. Tasked with devising a tailored care strategy in conjunction with the patient's doctor, various healthcare providers, and the patient's family, the aim is to guarantee the safety and well-being of clients.
Responsibilities:
Provides aid to individuals and directs them to appropriate resources and treatment options within the healthcare system to support the advancement of their treatment.
Observes patients to ensure they are getting the best care possible, working together with staff members throughout the institution to explore various treatment choices for the individual, enabling everyone to reach a well-informed conclusion.
Records are completed and documented to include details of screenings, referrals, and the care plan, ensuring that information is accurately stored for the benefit of both the individual and the broader healthcare system.
Collaborates with families and healthcare providers to create a comprehensive care strategy that guarantees the patient's well-being and safety.
Connects patients and their loved ones with local organizations for social assistance, community health resources, and various healthcare programs like Medicare and Medicaid to pave the way for ongoing patient care, particularly for individuals facing financial challenges.
