The main responsibility of the Clinical Social Worker II is to provide social work services to patients and their families, ensuring that their psychosocial and clinical needs are met comprehensively. Responsibilities:

Establish an individualized treatment plan for patients with limited guidance from clinical personnel to optimize their therapeutic outcomes.
Acting as a liaison, they educate staff, patients, and families on vital care-related information, aiming to improve comprehension within the caregiving dynamic.
Supplies crisis support and guidance in high-pressure circumstances for team members, relatives, and patients, employing clinical knowledge to provide consolation to all parties, offering insights that help alleviate the concerns of patients and their families.
Provides clinical care services for patients and their families, meeting their psychosocial and medical needs in the present and future.
