Overview:
In the role of Clinical Social Worker II, individuals provide social work assistance to patients and their families, aiding in the fulfillment of psychosocial and clinical requirements. By leveraging their expertise and background, this position identifies and tends to the concerns of patients, both within the clinical environment and in external contexts, employing therapeutic interventions and other clinical methods.
Responsibilities:
Determining the patients to prioritize for clinical social work intervention helps create an environment where staff and patients can benefit from the necessary social work support.
Reviews input from both the patient and healthcare team to comprehend the patient's circumstances and proficiently shares this data with all involved parties to mitigate issues stemming from gaps in information.
Utilizes clinical insight and experience to link patients to community resources and health-related opportunities, striving to deliver the best possible care for individuals.
Create an individualized care plan for patients with minimal guidance from clinical personnel to ensure they receive appropriate and effective medical interventions.
Serves as a bridge of knowledge between staff, patients, and families to ensure a comprehensive understanding of essential care-related details, facilitating smoother communication among all involved individuals.
Delivers critical support and direction in high-stress scenarios for staff, family members, and clients through the application of clinical proficiency to alleviate tensions and offer explanations that assist in calming the concerns of patients and their families.
Delivers clinical care to cater to the psychosocial and medical needs of patients and their families, both now and in the future.
Undertakes psychosocial evaluations of patients to gain insight into their situations and facilitate appropriate care recommendations aligned with the individual's medical and social well-being.
Utilizes diverse counseling and therapy strategies to aid in easing the patient and their family's concerns regarding clinical and medical tasks.
Executes supplementary responsibilities as delegated.

- Minimum of two (2) years of recent experience in a health-related environment
