Overview:
In long-term care facilities, the LPN LTC holds the responsibility of providing specialized medical attention to patients. Operating with moderate supervision, this position evaluates, devises strategies, and enacts measures aimed at fostering optimal recuperation and averting complications. Cooperation with various healthcare professionals is crucial for maintaining quality care standards and upholding wound care guidelines.
Responsibilities:
- Assesses the diverse needs of patients to formulate personalized care plans.
- Conducts treatments following approved practices and procedures.
- Monitors and appraises the development of patients, precisely documenting observations and communicating any modifications or apprehensions to the healthcare team.
- Imparts knowledge to patients, families, and caregivers on the correct ways to provide care, preventive measures, and symptoms indicating complications.
- Collaborates with physicians, registered nurses, and other healthcare professionals to coordinate care, ensure continuity, and optimize patient outcomes.
- Maintains precise and up-to-date records of assessments, interventions, and outcomes within the electronic health record platform.
- Undertakes other assigned obligations.
Qualifications:
EDUCATION:
Required:
High School Diploma or equivalent; Diploma in Practical Nursing.
EXPERIENCE:
Preferred:
1 year acute care experience
LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required:
Current LPN license in North Carolina; BLS (CPR)-American Red Cross or AHA Healthcare Provider correct
