Job Description
The Patient Placement Coordinator is tasked with arranging patient placements within the organization's facilities, working closely with others to streamline the process of bed assignments and transfers for improved efficiency.
Essential Job Function:
- Matching patients to specific bed assignments based on their clinical status and physical well-being, ensuring they are placed in appropriate locations per bed availability and allocation standards.
- Aids in the supervision of the Emergency Department's track board to facilitate the management of patient admissions, focusing on bed availability and patients in need of immediate care.
- Collaborates with healthcare professionals, including nurses and physicians, both internally and externally, to acquire necessary patient information crucial for placement decisions.
- Coordinates with RNs in charge of patient placement to address clinical inquiries by incorporating a clinician's viewpoint and ensuring alignment with the present bed availability status.
- Engages in regular communication with the Environmental Services team to oversee and organize the cleaning schedules for rooms, ensuring a swift transition for new patients seeking accommodation.
- Carries out supplementary responsibilities as delegated.
Education
- Required: High School Diploma or equivalent
