Job Description
The Case Manager RN offers comprehensive and harmonized care to patients, under the supervision of moderate to general oversight. This position involves evaluating patient requirements, participating in care coordination practices, conducting research, consolidating information, and reviewing care strategies.
Essential Job Function
- Reviews patient instances to gauge their needs and devise tailored care programs.
- Manages and reviews patient care documents to ensure accuracy and adherence to compliance regulations.
- Consistently updates knowledge through research on evidence-based practices and healthcare guidelines related to the assigned patient population, integrating the latest findings into the development of care plans and interventions.
- Critiquing and gauging the efficiency of care schedules, supervising patient results, and revising approaches as necessary to improve patient health and attain desired results.
- Carries out supplementary assignments as delegated.
Education
- Required: Associate's DegreePreferred: Bachelor's Degree
Experience
- Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
