Job Description
The Clinical Social Worker II is responsible for delivering social work support to patients and their families to address psychosocial and clinical needs effectively. Utilizing their professional skills and background, this role identifies and caters to the requirements of patients within and outside the clinical realm using therapeutic and other clinical methodologies.
Essential Job Function:
- Deciding on the patients to prioritize for clinical social work intervention is essential to ensure that staff and patients receive the necessary support to create a conducive environment.
- Assessing the details provided by both the patient and healthcare professionals is crucial in gaining insights into the patient's circumstances and communicating this effectively to all individuals involved, ultimately reducing the likelihood of issues resulting from inadequate information.
- Utilizes clinical experience to refer patients to community resources and health initiatives for enhanced quality of care.
- Establish an individualized care plan for patients, requiring minimal involvement from clinical personnel, to ensure optimal treatment outcomes.
- Teaches staff members, patients, and their relatives key information about care matters, facilitating improved understanding of the circumstances and acting as a mediator.
- Offers crisis intervention and direction during moments of high stress for staff, families, and individuals utilizing clinical proficiency to provide solace to all participants, delivering explanations that assist in calming the anxieties of individuals and their families.
- Delivers clinical care services to individuals and their families, ensuring that their psychosocial and medical needs are met effectively.
- Administers psychosocial assessments to grasp patients' backgrounds effectively, enabling the identification of suitable care options aligned with their medical and social well-being.
- Uses diverse counseling and therapy methods to create a sense of calm and confidence for the patient and their family in relation to clinical and medical interventions.
- Executes supplemental responsibilities as directed.
Education
- Required: Masters of Social Work
Experience
- Required: Minimum of two (2) years of recent experience in a health-related environment
Licensure/Certification/Listing
- Required: Licensed Clinical Social WorkerRequired: BLS certification
