Overview:
The Supervisor Advanced Care Transitions is responsible for managing the daily operations of the advanced care transition team to guarantee patients receive appropriate care as they move between different care settings. This role involves overseeing and organizing the team's activities such as recruitment, quality control, onboarding, coaching, skill evaluations, task allocations, and monitoring campus-specific results.
Responsibilities:
Oversees the advanced care transition team to ensure patients receive appropriate care tailored to their specific needs and age group as they transition between healthcare settings.
Acknowledges the importance of frontline staffing duties, identifying possible problems and notifying the relevant personnel to avoid coverage disruptions in advance.
Offers advice and support to all individuals in the Advanced Care Transitions group on a regular basis, addressing both routine challenges and intricate scenarios to enhance the overall efficiency of the team by implementing the most effective methods in Advanced Care.
Actively seeking out opportunities to enhance policies and procedures to boost effectiveness, questioning existing norms in case they hinder the organization's operational flow.
Examines issues that emerge within the department, delving deep to identify the fundamental cause in order to eradicate the problem and provide training and guidance to prevent its recurrence among staff.
Stays informed about current laws and guidelines to ensure that the Advanced Care Transition team adheres to local, state, and national regulations.
Carries out additional tasks as required.
