Job Description
The responsibilities of a Case Manager Registered Nurse (RN) encompass administering thorough and synchronized patient care. With adequate supervision, this position evaluates patient necessities, engages in care synchronization, conducts inquiries, reconciles data, and evaluates care blueprints.
Essential Job Function:
- Examines patient instances to evaluate their necessities and formulate personalized treatment blueprints.
- Arranges and manages documentation pertaining to patient health with a focus on accuracy and meeting regulatory guidelines.
- Keeping abreast of evidence-based practices and healthcare recommendations specific to the designated patient demographic enables the integration of cutting-edge knowledge into treatment plans and actions.
- Examines and judges the efficiency of treatment plans, tracking patient results, and adapting interventions as necessary to enhance patient health and reach desired targets.
- Executes any other responsibilities as delegated.
Education
- Required: Associate's Degree Preferred: Bachelor's Degree
Experience
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
