Job Description
The Community Health Worker in a school setting is responsible for supporting patients and their families in connecting with necessary healthcare services, community resources, and fostering positive lifestyle choices.
Essential Job Function
- Tasked with contacting identified patients, collaborating with individuals in the local community, in personal and group capacities, using diverse communication methods such as direct interaction to foster health-focused programs and wellness strategies.
- Engages with the patient's care team to guarantee that the patient is linked to services that enhance their overall health and aids in establishing a relationship with a primary care provider and arranging suitable appointments.
- Engages in cooperation with colleagues to develop a collection of resources for the community and healthcare sectors.
- Offers support to patients in understanding and addressing challenges preventing them from accessing necessary healthcare services.
- Engages in team discussions to give feedback and suggestions on patient necessities and productive methods to cater to them.
- Compiles and aids in the examination of data sourced from Cone Health and public resources to pinpoint and address care gaps and health inequities.
- Conducts screenings to identify patients' social determinants of health and facilitates connections to suitable services, empowering patients to recognize obstacles, establish objectives, and advance their well-being.
Education
- Required: Completion of High School Diploma/GED.
- Preferred: Completion of North Carolina Community Health Worker Standardized Core Competency Training Program (NC CHW SCCT) .
Experience
- Required: 2 years of experience in Care Management or equivalent university studies course work. Must live in or have a personal connection to the community being served or lived experience of health disparities in family or community.
- Preferred: Experience working in healthcare setting and experience health coaching within historically marginalized populations.
Licensure/Certification/Listing
- Required: Valid Driver s License and must have reliable transportation to, from and between locations. Eligibility for listing as a NC Certified Community Health Worker required within one year of hire also is required.
