Job Description
The RN Care Management Lead plays a crucial role within the Case Management team, overseeing the care of patients with notable medical and psychosocial requirements, who are at a heightened risk of re-admission or extended hospital stays. Working closely with the Care Management staff, the Lead offers specialized assistance for intricate cases at the hospital and spearheads the coordination efforts across the system through Complex Case Rounds. This team serves as a vital link between hospital care and various outpatient or service-oriented teams to guarantee seamless transitions and minimize unnecessary readmissions for patients with the most critical needs.
Essential Job Function:
- Identification and assessment of patients throughout the hospital who have prolonged stays and complex care requirements are essential for the unit's case management team to provide the necessary time and coordination.
- Utilize Cone's EPIC predictive analytics to pinpoint and oversee potential high-risk patients or those marked with a challenging predictive indicator for Care Planning. Create and execute thorough, individualized discharge strategies that prioritize the patient's needs and ensure seamless care transitions while optimizing resource allocation through advanced clinical decision-making and innovative issue-solving techniques.
- Facilitating inpatient interdisciplinary Complex Case Rounds as a team effort could enhance coordination and collaboration in addressing complex patient cases.
- Collaborate with emergency department staff, social workers, and case managers to interact with challenging patients in the emergency department, in order to find suitable alternative care options that can help achieve the best results.
- Work together with ambulatory and community-based groups to guarantee smooth handovers, reduce readmissions, and arrange for post-discharge check-ups to support a successful move to the next care stage.
- Establish and nurture robust connections with post-acute care facilities, insurance case managers, primary care provider case managers, and VBC Initiatives to facilitate the seamless provision of value-based care services.
- Work together with Cone's UR/UM department to guarantee that patients have the freedom to choose their care options while also making sure that post-acute partners are in agreement to ensure the best possible transition to the next level of care is achieved. Offer support in creating a financial evaluation for patients with long length of stay who have complex medical and social needs and advocate for the essential resources needed for a successful discharge.
- Inform patients and their families about available care choices, requirements after leaving the hospital, and local support services to ensure ongoing care.
- Observing the management of unnecessary days and preventable delays in a workload to pinpoint obstacles in the system and reveal chances for enhancing processes within hospital functions and collaborations within the community.
- In charge of tasks like introducing new employees to the organization, conducting training sessions for specific departments, serving as an authoritative figure and support for colleagues, taking part in recruitment interviews, being available for urgent matters, assigning work coverage, ensuring safety measures are implemented, and attending key hospital meetings on behalf of the department.
Education:
Required: Master's degree from accredited school of Social Work.
Experience:
Required: 5 years inpatient case management experience.
Preferred: Experience in people management. ACM-SW or CCM certified with 3 years of inpatient case management experience.
Licensure/Certification/Listing:
Required: LCSW in the state of NC, required BLS/CPR.
