The Care Guide offers assistance to a specific set of patients in understanding and managing their treatment process within the institution. This position acts as a representative for patients, collaborating with the main healthcare team to enhance the standard of care and wellness results for patients. Additionally, the Care Guide ensures that healthcare providers and staff are performing at their highest level of proficiency while adhering to their authorized scope of practice.
Responsibilities:
Offers support in organizing appointments for patients by contacting individuals with scheduled appointments to guarantee punctual attendance, thereby optimizing overall office operations.
Coordinates transportation services for patients in need to ensure they can access essential medical appointments, contributing to the overall well-being of both individual patients and the community.
Acts as an advocate for patients, ensuring that the primary care team is delivering the best possible care.
Collaborates with the primary care team to facilitate patient's access to community and network health resources.
Effectively communicates in a compassionate manner with patients, families, and healthcare providers to enhance comprehension and guarantee a positive interaction with the institution.
