Job Description
Overview:
The Clinical Social Worker II offers social work support to patients and their families to address their psychosocial and clinical requirements. By leveraging their knowledge and skills, this position recognizes and caters to the concerns of patients in various settings, implementing therapeutic methods and clinical approaches.
Responsibilities:
Identifying the patients who require focused attention from clinical social workers ensures that both staff and patients receive suitable social work assistance within the proper setting.
Evaluates data from both the patient and medical personnel to gain insight into the patient's condition and effectively conveys this information to ensure a comprehensive understanding and reduce the likelihood of gaps causing problems.
Utilizes clinical expertise and experience to connect patients with local resources and health programs to guarantee optimal care is provided.
Developing a care strategy for patients with limited help from medical personnel to facilitate access to personalized treatment.
Educating both employees and patients along with their families on essential care-related information acts as a bridge, facilitating a better grasp of the circumstances for all involved.
Offers assistance during critical situations and takes charge in high-stress scenarios to provide support to staff, family members, and patients. Draws on clinical knowledge to calm all individuals involved, offering clear explanations to alleviate the concerns of patients and their loved ones.
Offers clinical support to patients and their families in addressing present and future psychological and healthcare requirements.
Psychosocial assessments are conducted for patients to gain insight into their circumstances so they can be connected with the most appropriate care option based on their medical and social conditions.
Utilizes a range of counseling and therapy methodologies to create a sense of comfort for the patient and their family in relation to clinical and medical procedures.
Carries out additional responsibilities as required.

EXPERIENCE:
Required:
- Minimum of two (2) years of recent experience in a health-related environment
