Job Description
A Case Manager Registered Nurse (RN) is accountable for providing holistic and harmonized patient care services. Under controlled supervision, this role assesses patient needs, engages in care harmonization, undertakes research activities, aligns information, and examines care schemes.
Essential Job Function
- Assesses patient situations to understand their requirements and establish unique care arrangements.
- Manages and deals with paperwork associated with patient care to meet accuracy standards and regulatory compliance.
- Continuously researching and remaining informed about evidence-based strategies and healthcare standards pertinent to the assigned patient cohort facilitates the integration of the most recent insights into care plans and interventions.
- Observes and assesses the success of healthcare plans, monitoring patient advancements, and adjusting strategies as necessary to enhance patient health and accomplish specific goals.
- Undertakes supplementary duties as directed.
Education:
- Required: Associate's degree.
- Preferred: Bachelor's degree.
Experience:
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification/Listing:
- Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider.
