Job Description
The Behavioral Health Coordinator/Bachelor of Social Work (BSW) provides social work assistance to patients and families dealing with mental health and substance use disorders, life stressors and crises, and stress-related physical symptoms. This role assists with the prevention, diagnosis and treatment of these conditions. Working under close supervision, this role advocates for patients by helping them access resources and coordinating care after discharge.
Essential Job Function:
- Recognizes and taps into resources to improve the successful and productive outcomes of patient behavioral care in partnership with other members of the multidisciplinary team.
- Assists in coordinating post-discharge care for patients and their families across multiple systems, such as hospitals, educational institutions, social services, and community resources.
- Aids patients and their families in managing personal and environmental obstacles that can increase the likelihood of illness or obstruct the patient's ability to receive optimal care.
- Stays informed about community health and social agencies to broaden the behavioral health services accessible to patients through cooperation or direction.
- Supports the patient and their family in grasping medical suggestions and delivering interventions designed to help the patient achieve optimal social and health adaptations.
- Offers support with documentation and guiding patients to appropriate post-discharge services.
- Executes any other responsibilities as delegated.
Education
- Required: Bachelor's in Social Work or related field
Experience
Licensure/Certification/Listing
- Required: BLS (CPR)-American Red Cross or AHA Healthcare Provider
