Job Description
The LPN LTC is accountable for furnishing specialized assistance to patients undergoing long-term care, taking charge of assessing, strategizing, and carrying out interventions to encourage optimal recovery and mitigate risks. Engaging with a multidisciplinary team is essential in this role to secure the provision of excellent care and adherence to the most effective wound care techniques.
Essential Job Function
- Conducts detailed evaluations of patients' needs to establish tailored care plans.
- Carries out treatments in accordance with established practices and protocols.
- Reviews and assesses patients' development, precisely documenting observations and conveying any changes or worries to the healthcare team.
- Instructs patients, families, and caregivers on the correct care protocols, methods to prevent issues, and symptoms indicating possible complications.
- Partners with doctors, registered nurses, and other healthcare providers to align care, maintain consistency, and enhance patient outcomes.
- Maintains precise and updated documentation of appraisals, interventions, and consequences in the electronic health record platform.
- Completes any other assigned responsibilities.
Education
- Required: High School Diploma or equivalent; Diploma in Practical Nursing.
Experience
- Preferred: 1 year acute care experience
Licensure/Certification/Listing
- Required: Current LPN license in North Carolina; BLS (CPR)-American Red Cross or AHA Healthcare Provider correct
