Overview:
The Physical Therapist II evaluates the conditions of patients, formulates tailored treatment plans, and enacts therapeutic measures to enhance their functional capabilities. This role is skilled at efficiently treating all patients in the clinical setting and fulfilling all physical therapy tasks with little supervision. Moreover, this role initiates conversations among team members from various disciplines, provides competent care to familiar patients, and advocates for both patients and the team. When faced with complex situations, this position may necessitate mentoring and could be tasked with providing coverage in other settings.
Responsibilities:
Evaluates and treats a diverse array of diagnoses and complex patients.
Reviews patient's physical aptitudes, medical histories, and functional limitations to craft individualized treatment programs.
Executes therapeutic interventions and conducts physical therapy exercises aimed at improving patient's strength, flexibility, and range of motion.
Teaches patients and their families about treatment approaches, home-based exercises, and self-management skills to facilitate better recovery results.
Partners with peers to establish and implement discharge plans and suggestions for continuous care.
Keeps track and documents the advancement of patients, treatment outcomes, and reactions to interventions to support the adaptation of treatment strategies when necessary.
Partners with colleagues from various departments to facilitate coordinated patient care and holistic treatment success.
Undertakes tasks beyond the regular scope of work.
