Job Description
The Case Manager Registered Nurse (RN) administers comprehensive and harmonized patient support. While receiving moderate to general supervision, this individual examines patient needs, collaborates on care, conducts research, aligns information, and assesses care arrangements.
Essential Job Function:
- Examines patient situations to evaluate their requirements and create tailored care blueprints.
- Compiles and manages paperwork connected to patient care to uphold accuracy and conformity with regulatory guidelines.
- Keeps abreast of evidence-based approaches and medical directives relevant to the designated patient cohort to apply the latest understanding to patient care plans and treatments.
- The effectiveness of care regimens is appraised, patient outcomes are closely observed, and adjustments are made when necessary to promote patient wellness and accomplish desired goals.
- Undertakes supplementary duties as needed.
Education:
- Required: Associate's DegreePreferred: Bachelor's Degree
Experience:
- Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
