Job Description
This position focuses on catering to patients' needs by delivering therapeutic and clinical methods both within and beyond the healthcare environment.
Essential Job Function:
- Assists in identifying the specific individuals that would benefit from dedicated clinical social work interventions, thereby facilitating a conducive environment for both patients and staff, supplemented with the required social work support.
- Obtains details from both the individual receiving care and the healthcare team to facilitate smooth communication among all involved, minimizing the risk of any obstacles or information discrepancies that could disrupt the clinical process.
- Connects patients with healthcare providers as well as community resources to offer the most suitable support for their well-being, prioritizing the delivery of high-quality care that is essential for their health.
- Coordinates with other healthcare professionals to devise personalized care plans for patients, catering to their specific needs.
- Informing staff members, patients, and their families about crucial care-related information to enhance overall awareness and serve as a valuable bridge between all parties involved.
- Provides support during high-stress incidents for staff, family, and patients by utilizing clinical expertise to help calm all parties involved in these situations that often arise in clinical settings.
- Executing further responsibilities as designated.
Education:
- Required: Master's of Social Work.
Experience:
- Required: Minimum of two (2) years of recent experience in a health-related environment.
Licensure/Certification/Listing:
- Required: Licensed Clinical Social Worker Associate.
- Required: BLS certification.
