Job Description
Overview:
The Community Health Worker operates within specified communities to assist individuals and their families in accessing necessary medical and healthcare services, community support, and embracing healthy habits
Responsibilities:
Responsible for engaging with specified individuals, interacting with patients within the community through both one-on-one and group sessions, utilizing various communication channels such as face-to-face meetings to promote and support health initiatives.
Collaborates with the team of healthcare professionals to ensure that patients are linked to resources that promote their well-being and assists them in establishing a relationship with a primary care provider and scheduling appointments as necessary.
Works together with fellow team members to establish a collection of resources for both the community and healthcare sectors.
Supports individuals in maneuvering through the healthcare system, assisting them in recognizing and overcoming obstacles to receiving healthcare services.
Engages in team meetings by offering insights and suggestions on patient requirements and effective approaches to meet them.
Assists in gathering and examining information from Cone Health and public datasets to pinpoint and rectify deficiencies in healthcare access and tackle inequalities in health outcomes.
Assesses patients for social needs impacting their health and links them to resources, guiding them in self-discovery, goal setting, and health improvement
Qualifications:
EDUCATION:
Required:
- Completion of High School Diploma/GED.
Preferred:
- Completion of North Carolina Community Health Worker Standardized Core Competency Training Program (NC CHW SCCT)
EXPERIENCE:
Required:
- 2 years of experience in Care Management or equivalent university studies course work. Must live in or have a personal connection to the community being served or lived experience of health disparities in family or community.
Preferred:
- Experience working in healthcare setting and experience health coaching within historically marginalized populations.
LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIRED
Required:
Valid Driver's License and must have reliable transportation to, from and between locations. Eligibility for listing as a NC Certified Community Health Worker required within one year of hire also is required.
