Job Description
Tasked with providing holistic and well-coordinated care, the Case Manager Registered Nurse (RN) operates under moderate to general monitoring. They analyze patient necessities, participate in organizing care, conduct inquiries, ensure information coherence, and review care blueprints.
Essential Job Function
- Examines patient situations to evaluate their needs and devise tailored care arrangements.
- Prepares and handles documentation concerning patient care to guarantee compliance with regulations and accuracy.
- Keeping abreast of the latest evidence-based practices and healthcare guidelines pertaining to the assigned patient population allows for the incorporation of up-to-date knowledge into interventions and care plans.
- Evaluates and examines the efficacy of care schedules, overseeing patient advancements and adjusting as necessary to improve patient wellness towards achieving intended aims.
- Undertakes supplementary duties as directed.
Education
- Required: Associate's Degree Preferred: Bachelor's Degree
Experience
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations
Licensure/Certification/Listing
- Required:Registered Nurse license in North Carolina or a Compact stateBLS (CPR)-American Red Cross or AHA Healthcare Provider
