Overview:
The Physical Therapist II is responsible for evaluating patient's conditions, devising individualized treatment programs, and executing therapeutic interventions to enhance their functional capabilities. This role is competent in managing all patients at their practice facility efficiently and confidently carries out all physical therapy responsibilities with minimal supervision. Additionally, this role fosters communication among team members from different disciplines, provides experienced care to familiar patients, and acts as an advocate for both patients and the team. In challenging situations, this position may require guidance and may be asked to cover duties in alternate settings as needed.
Responsibilities:
Provides evaluation and treatment for various diagnoses and intricate patients.
Evaluates patient's physical capabilities, medical records, and limitations in functionality to create customized treatment plans.
Delivers prescribed exercises and various interventions in physical therapy to enhance the strength, flexibility, and mobility of individuals undergoing treatment.
Provides information to patients and their families about recovery strategies, home workout routines, and self-care practices to boost treatment effectiveness.
Engages in collaboration with colleagues to formulate and carry out discharge plans and advice for sustained healthcare.
Watches over and records the development of patients, the results of treatments, and how they react to interventions to allow for treatment plans to be adapted accordingly.
Engages in teamwork across different departments to promote integrated patient care and holistic treatment effectiveness.
Executes any other responsibilities given.
