Job Description
Overview:
The Case Manager Registered Nurse (RN) administers comprehensive and harmonized patient care. Operating under moderate or general supervision, this role assesses patient needs, engages in care planning, conducts inquiries, aligns information, and reviews care strategies.
Responsibilities:
- Assesses patient situations to understand their needs and devise customized care blueprints.
- Organizes and manages documentation linked to patient care to ensure accuracy and regulatory compliance.
- Remains current on evidence-based practices and healthcare protocols relevant to the assigned patient demographic, integrating new knowledge into treatment strategies and plans.
- Examines and assesses the efficacy of care schedules, overseeing patient advancements and adjusting where necessary to enhance patient health with the goal of achieving desired outcomes.
- Performs supplementary obligations as instructed.
- Qualifications:
Education:
- Required: Associate's degree.
- Preferred: Bachelor's degree.
Experience:
- Required: 2 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations.
Licensure/Certification/Listing:
- Required: Registered Nurse license in North Carolina or a Compact state BLS (CPR)-American Red Cross or AHA Healthcare Provider.
