Overview:
The Case Manager Registered Nurse (RN) is tasked with delivering complete and coordinated care to patients. With varying levels of supervision, this position evaluates patient needs, collaborates in care coordination, conducts research, aligns information, and assesses care plans.
Responsibilities:
Studies patient cases to assess their requirements and formulate individualized care blueprints.
Handles and processes paperwork related to patient care to ensure precise documentation and adherence to regulatory obligations.
Consistently updates understanding of evidence-based practices and healthcare standards specific to the assigned patient group to optimize the quality of care plans and interventions.
Examines and appraises the efficacy of care schemes, keeping track of patient advancements, and making alterations as necessary to optimize patient health for the achievement of desired goals.
Performs any other assignments as necessary.
